KEY RESPONSIBILITIES
- Processing new referrals, including data entry of client information;
- Responding appropriately to incoming phone calls and inquiries as directed by triage decision making tree;
- Updating and maintaining current client information and records in CHRIS database, including client’s service bookings;
- Assembling and maintaining files of pertinent client information; verifying that all relevant documents have been completed and filed;
- Booking home visits and client conferences as needed;
- Processing letters, client applications and other client forms;
- Preparing client reports for Care Coordinators/Placement Coordinators as required;
- Generating reports and labels from CHRIS;
- Securing and/or shredding confidential documents as required;
- Transcribing, copying and distributing letters, memos and conference reports using word processing software and other equipment;

EDUCATION AND EXPERIENCE
- Successful completion of grade 12, plus an additional program of up to one year in duration;
- Two to three years of relevant administrative office experience;
- Knowledge or experience in medical terminology an asset;
- Understanding of available community resources, services and the roles of health care professionals, hospital and support workers;
- Proficient in the operation of a personal computer in a networked environment using computer software and data entry skills;
- Able to communicate effectively in both oral and written formats with internal staff and external clients;
- Knowledge of office systems, procedures and business practices;
- Able to work effectively in a team based setting, strong planning and organizational skills, along with the ability to multi-task and respond to changing priorities, and to work independently;
- Able to relate effectively with sensitivity, tact and discretion to clients, family members, staff and the public, along with ability to maintain confidentiality.

KEY RESPONSIBILITIES
- Answering and screening initial incoming phone calls according to appropriate policy & procedures; while striving to minimize the number of subsequent transfers and contacts;

EDUCATION AND EXPERIENCE
- Minimum two years previous experience in similar or related position preferably in the health, social or community service field.
- Two – three year post-secondary diploma preferably in health, social or community services, or equivalent combination of education and experience.
- Certified Information & Referral Specialist is a requirement or commitment to obtain certification within a 1 year period; medical terminology required.
- Proficiency in French considered an asset.
- Proficient in the operation of a personal computer in a networked environment using computer software, data entry skills and keyboarding skills.
- Ability to conduct information searches using multiple resources; e.g. internet, data bases, etc.
- Proficient with various software programs and databases, including Access, to generate reports and create presentations.

POSITION SUMMARYReporting to the Patient Services Manager, this self-directed position functions in an advanced nursing role within a collaborative practice model. The Nurse Practitioner (NP) will support individuals receiving the services of Home and Community Care Support Services (HCCSS) Waterloo Wellington who may not have a family physician and require Nurse Practitioner support, while others may have a family physician who is requesting the advanced wound care knowledge, assessment and support of the NP. The NP acts as a key resource to all HCCSS and service provider staff regarding wound care and acute or chronic health conditions.EDUCATION AND EXPERIENCE
- Current registration with the College of Nurses of Ontario
- Registered Nurse in the Extended Class (RN EC designation)/expectation to have RN EC registration and successful completion of the Nurse Practitioners’ exam set by the College of Nurses of Ontario
- Advanced wound care certification such as NSWOC, IIWCC, MCISc-WH or equivalent
- Advanced experience in wound care practice including equipment and product selection
- Familiarity with the services available within the community sector
- Demonstrated expertise in wound care and collaborating with external agencies and community partners
- Advanced assessment skills and sound knowledge of clinical therapeutics
- Demonstrated clinical leadership and collaborative practice with all care providers
- The ability to travel throughout Waterloo Region and Wellington County is required.
- Fluency in French language preferred.

Position Summary

Responsible for participation in establishing and managing the contract management framework for contracted service providers and vendors. Provides leadership/mentorship for patient services, quality risk colleagues, and service providers on contractual, service related and/or client issues to de-escalate and mitigate risk. Accountable for implementation and sustainability of new initiatives and programs including revising and developing supporting policies and practices. Provides expert advice to internal and external stakeholders on performance, accountability and patient care procurement matters as required. Demonstrates excellent skills and abilities to develop effective working relationships, provide leadership and build effective teams with both internal and external stakeholders. Also provides project management, direction and leadership to all HCCSS staff as appropriate and direct supervision/mentorship of designated members of the team.

What will you do?
- Supports the Director, Patient Care Contract Management in instituting local and provincial administrative policies and practices for the ongoing management and oversight of existing contracts, including but not limited to, volume and market share management, contract performance management, and initiation of corrective contractual remedies when necessary to ensure safe, quality care for patients
- Contributes to analysis of local and regional system performance within the context of provincial performance goals, and assesses the implications for management of contracted service provider and vendor performance targets
- Reviews and validates performance reports, determines performance against contract specifications and makes recommendations to address emerging issues or requirements for remedial action in consultation with the Director
- Effective management of risk at HCCSS by applying expertise/leadership in the identification, assessment, mitigation and prioritization of perceived and/or actual patient and sector risk situations
- Maintains oversight of contract CHRIS database to meet contractual obligations
- Leads and/or collaborates with internal and external stakeholders in quality improvement initiatives
- Provides leadership to staff reporting to them as required, and monitors their work, and priorities during projects and assignments; this includes ensuring adherence to maintaining all appropriate and required documentation and record keeping
- Other duties as assigned, including supporting various committees and working groups in achieving the goals of HCCSS and the ministry
- Performs on-call support to operational divisions after hours if required

What must you have?
