Are you an experienced registered nurse (BScN or diploma) looking for a different kind of practice environment, and comfortable practising both independently and as part of a team? This could be what you’ve been looking for.   As an integral part of our Rapid Response Nursing (RRN) team, you will work with medically complex children and/or frail adults and seniors with medically complex needs and/or high-risk characteristics such as congestive heart failure, to ensure a smooth transition from acute care to home care. You will achieve this in two ways: by connecting with primary care and by providing hands-on rapid response home care.   In fact, you’ll make the first in-home nursing visit to high-needs patients, shortly after their discharge from hospital, during which you’ll confirm the hospital discharge care plan, emphasize the importance of primary care to avoid re-hospitalization, and reconcile medications. What will you do?   Reporting to the Director, Integrated Care, as an integrated member of the Palliative Care Team, the Nurse Practitioner (NP) supports the Home and Community Care Support Services Mississauga Halton Palliative Program and the Home and Community Care Support Services Mississauga Palliative Care Regional Programing to ensure high quality, patient-centered hospice palliative care across all care settings in the Mississauga Halton region.  The NP provides support to the Palliative Care Team and specialist providers to address palliative care needs and increase capacity within the region to provide palliative and end of life care in the community. Critical to the NP role is the provision of direct nursing services to support patients with palliative needs and their families. The NP broad scope practice will support seamless integration of hospice palliative care at the primary, secondary and tertiary care level; serving as clinician, educator, collaborator and advocate. At the primary level, the incumbent works as an integral member of the Palliative Care Team, assisting in building team capacity to provide care and clinical management; at the secondary level, engaging in shared care with primary and specialist providers, to address more complex palliative care needs; and at the tertiary level facilitating access to and seamless transition to and from tertiary services.     Working collaboratively across the health care system, the NP provides expert direct clinical palliative care and leadership to support seamless, integrated care delivery. In this leadership role, the NP will work in all domains consistent with advanced practice nursing (leadership, clinical care, consultation/collaboration, facilitation and research). Within this scope, the NP will provide home/office/hospital visits and telephone and face-to-face consultation, and crisis response, The NP is also responsible for education, knowledge transfer and best practice implementation, mentorship and professional development, through coaching inter-professional teams involved in the circle of support.   The role includes stakeholder engagement, participation in local, regional and provincial committees and a leadership role to implement improvements in Home and Community Care Support Services Mississauga Halton and within the Mississauga Halton Palliative Care Network. The Community Transition Nurse (RN/RPN) provides individualized patient care through a holistic and collaborative approach, working closely with the High Intensity Supports at Home (HISH) team, patient/family and primary care provider to develop a unique plan of care specific to their needs.   NATURE & SCOPE: - Assists with hospital discharge and/or environmental transition of high-risk seniors, ensuring the patient is seen in home within 24-48 hours of being admitted to the HISH Program. The RPN will consult with the RN or assigned Care Coordinator when support is required for complex discharges or transitions.  - Coordinates with HISH or assigned Care Coordinator in the development of care plan necessary to provide patient with wrap-around, holistic nursing support to remain safely at home while waiting for LTC placement. - Provides ongoing, short-term support to patients’ through education and coaching, clinical assessment, assistance with medication management, and communication with primary care to reduce hospital admission/readmission. This may include daily visits to ensure patients’ needs are being met. Responsibilities will include:  assisting school boards in recognizing and responding to student mental health and addiction issues; providing services and support to students with mild to complex mental health and substance abuse issues; developing plans for clients with mental health and addiction needs including the transition of students back to school from hospitalization; medication management and providing support or intervention for issues such as self-harm or treatment refusal.  The Mental Health and Addictions Nurse will also play a key role in helping students and/or parents’ access services such as family health and/or addictions agencies.  The position will be located within the Erie St. Clair Region.
