What will you do?

Reporting to a Senior Manager, the Placement Coordinator is responsible for reviewing client assessments, and eligibility, authorizing the admissions to Long Stay, Short Stay and Convalescent Care beds in Long Term Care Homes (LTCH) and the management of Long Term Care waitlists.

 

Key accountabilities include:

  • Obtains and reviews the application for long-term care in both the in home and hospital settings.
  • Complete assessments using RAI-HC.
  • Conduct Home visits to assess client service needs to determine eligibility for placement and in-home services.
  • Counsels older and/or disabled adults and their families on long-term care placement.
  • Ensures equitable access into appropriate homes following legislation, guidelines, processes and procedures.
  • Discusses alternative options for clients, where appropriate.
  • Authorizes admissions to LTCH.
  • Conducts bed offers.
  • Maintains waitlists for clients.
  • Ensures client services are provided in accordance with Home and Community Care Support Services Central East policies, legislation and professional disciplines.
  • Manages client problems and complaints.
  • Establishes and maintains effective relationships with service providers/LTCH to deliver quality client care.
  • Participates in community activities to promote Home and Community Care Support Services Central East objectives, activities, programs and services.
  • Establishes and maintains relationships with professional organizations to ensure quality of care knowledge is current.
  • Works effectively with management and colleagues to deliver effective and efficient client service delivery.
  • Assumes other duties as required.

What must you have?

  • Membership, in good standing, with the applicable regulatory body:
    • College of Nurses of Ontario (RN)
    • College of Physiotherapists of Ontario
    • College of Occupational Therapists of Ontario
    • Ontario College of Social Workers and Social Service Workers (RSW)
  • Working knowledge and understanding of applicable legislation, regulations, guidelines, processes and policies and procedures.
  • Sound knowledge of the Fixing Long-Term Care Act, 2021 and its related regulations.
  • Minimum 2-5 years direct experience in LTC and/or completing placement applications.
  • Working knowledge of RAI-HC and CHRIS.
  • Working knowledge of Home and Community Care Support Services Central East's role in the community sector.
  • Understanding of the integration and partnerships with hospital/community sector and an understanding of transitional issues between health care environments.
  • Demonstrated ability to use computer programs utilized by the Home and Community Care Support Services Central East.
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

 

What would give you the edge?

  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language 

 

Hours of work:

All hours of operation (Initially Monday-Friday 0830-1630; 70 hours per bi-weekly pay period) subject to change as per the Collective Agreement.

Position location and travel:

Hybrid work model, subject to change. 

 

Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario driver’s license and access to a vehicle are necessary.
