What will you do?

 

Reporting to a Manager, Patient Services, the Nurse Practitioner (NP) - Palliative is primarily responsible for providing leadership, clinical case consultation and direct clinical care to patients, and their family/caregivers, who require hospice palliative care within Ontario Health atHome. The NP will support and enhance the capacity of primary care practitioners providing hospice palliative care. 

 

  • Provide comprehensive health care services, including pain and symptom management, per the College of Nurses of Ontario (CNO) Practice Standards for Nurse Practitioners, for patients who require hospice palliative care to enhance quality palliative care
  • Is an integral member of the Palliative Care Outreach Team (PCOT)
  • Participate in continuing education and training initiatives at and across the Ontario Health atHome, including mentoring team members
  • Anticipate changing priorities and adjust as needed to meet overall needs of patients and their families
  • Embody Ontario Health atHome mission, vision and values and apply quadruple aim (enhancing patient experience, enhancing provider/staff experience, improving value and improving populations health) to support continuous quality improvement in daily work
  • Establish and maintain collaborative and effective working relationships with Ontario Health atHome palliative care system partners
  • Adhere to Ontario Health atHome patient safety policies and procedure

 

 What you must have?

  • Registration in the Extended Class from the College of Nurses of Ontario without restrictions, and therefore eligible to practice as a RN(EC)
  • Canadian Nursing Association Certification in Hospice Palliative Care or relevant specialty certification preferred
  • Master’s degree preferred
  • Evidence of Cultural Sensitivity and Cultural Safety training
  • Evidence of continuing professional development
  • 3 to 5 years’ experience in hospice palliative care. Professional experience in health care should include: linkages with community health agencies and other health disciplines, needs assessment, and case management and/or discharge planning OR
  • Nurse Practitioner with experience in primary care and/or acute care with an interest in developing an advanced level of knowledge and skill in hospice palliative care
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date

  • Strong consultation skills and their application with individuals, teams and with community service providers
  • Ability to research, analyze, and evaluate hospice palliative care best practices
  • Knowledge of local First Nations communities and Indigenous Peoples to support the provision of culturally safe care
  • Knowledge of the needs of the Francophone communities 
  • Fluency in English (oral and written).  Fluency in French preferred.

ONA Wage Grid: $62.06/Hr. - $74.11/Hr.
