As an Intake Coordinator (Status: Full-Time, Start Date: Immediately; Location: Sault Ste. Marie), you will complete assessments based on the patient’s complexity, predictability, and within the RPN scope of practice to determine the patient’s eligibility. You will plan, evaluate, and coordinate initial services required to address patient needs, and ensure the most effective use of resources.

What will you do?

  • In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans
  • Link patients with service providers
  • Coordinate and monitor care plan delivery
  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected

What must you have?

  • Registered Practical Nurse in good standing with the College of Nurses of Ontario.
  • Relevant experience in long term care homes or health care and/or in social services.
  • Experience working with seniors and/or physically disabled individuals.
  • Understanding of issues and priorities within the health care sector.
  • Knowledge of tools, systems, and databases used in health care service delivery and management.
  • Practical knowledge and understanding of relevant legislation (e.g. regarding the provision of health care services, privacy, etc.).
  • Effective planning, organizing, and evaluation skills to manage multiple patients and provide information reports upon request.
  • Ability to accurately complete required documentation, reports, forms, and recommendations.
  • Strong interpersonal and communication skills to work with diverse patient groups with varying levels of comprehension and language capability.
  • Effective listening and strong facilitation skills to work with patients and their families, caregivers, or substitute decision-makers (SDMs).
  • Effective communication skills to establish and maintain contacts with organizations within the community.
  • Ability to deal with demanding interpersonal situations and respond with good judgment and understanding.
  • Ability to communicate using various software programs to clearly articulate patient status.
  • Strong collaboration skills to participate in projects and committees within the organization.
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel, Power Point, Outlook, etc.).
  • A valid driver’s license and access to a reliable vehicle.
  • Advanced oral and written proficiency in English is essential.
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

What would give you the edge?

  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
