Ontario Health atHome is looking for an experienced Patient Services, Manager. This position provides leadership, supervision and support for the staff. The Manager is responsible for the assessment, planning, development, implementation and evaluation of systems, activities and goals of the Patient Services department, that support the corporate strategic directions and the division’s strategic priorities and plans.

What will you do?

  • Assists the Directors in the Patient Services division in the assessment, planning and implementation of Community Care departmental goals, activities, programs and services that support the Ontario Health atHome needs of the population across Northwestern Ontario.
  • Ensures high quality standards in the delivery of Ontario Health atHome; continuity of care provision and quality and safe client services are in sustained and in place.
  • Act as a key staff resource to the Directors, Patient Services.
  • Assesses community needs based on population health needs, plans, implements and develop community care goals that is responsive to the population health needs and identifies need for service changes  in conjunction with the staff and other key stakeholders cross sectors. 
  • Aligns annual work plans for operational work to the broader strategic directions of the Ontario Health atHome and the divisional strategy and priorities including monitoring of division performance indicators.
  • Ensure the department policies and procedures are adhered to through the supervision and management of the department staff.
  • Leads, guides and supports the Ontario Health atHome unionized and support staff in their work and professional development including issues management and resolution seeking guidance from the Director, Human Resources as appropriate.
  • Provides leadership to the department staff in Ontario Health atHome business processes as it relates to the enhanced role and expanding mandate regionally and based on provincial direction.
  • Liaise with external constituent groups, organizations and key stakeholders to assess population health needs, and recommends innovative services and programs that align and integrate at the regional level.
  • Keep abreast of leading practices in Ontario Health atHome in accordance with best practice standards, government/ministry directives and policies and procedures and advises the Director of changes and the impact of changes in Ontario Health atHome delivery.
  • Contributes to the financial planning and budgetary control of the department through effective planning and management of department business processes and by refinement of operating practices.
  • Provides operational leadership to the staff in the day-to-day business as it relates to Ontario Health atHome services and programs specifically:
    • Identification of population health needs and specific patient care needs
    • Explore options best suited to patient need
    • Determine the patient’s eligibility for government-funded services and setting
    • Work closely with the service providers and other stakeholders to integrate and coordinate care across the health care continuum
    • Identify how best to deliver services in collaboration with system partners
    • Participate in case conferences and case reviews
    • Participate in the client appeals process
    • Assess and support assessment, intake, referral and improved access to available government-funded Ontario Health atHome
    • Support applications to day programs, supportive housing or assisted living programs and certain complex continuing care or rehab bed services,
    • Knowledgeable about the application process and admission to long-term care
    • Assists and supports the management and integration of wait lists across the community support services sector
    • Monitor indicators related to wait times for service initiation, the patient care experience, continuity of care, and advises the Director, Patient Services on recommended improvements in practice or changes in business process.
  • Addresses equity in access to service, service quality, patient safety and patient privacy and works closely with departmental staff to achieve alignment in business processes and resolution of issues.
  • Identifies risks and mitigation strategies that relate to the provision of Ontario Health atHome.
  • Achieves and maintains maximum integration of activities across the Ontario Health atHome division.
  • Participates in the monitoring of the performance of the contracted service providers.
  • Plan and implement in conjunction with Human Resources on effective strategies in the recruitment, hiring, support and performance review and management of the staff.
  • Active contributing member of the management team.
  • Other duties as assigned.
  • Maintains high levels of confidentiality and discretion when working with confidential organizational, financial and human resources data, including details about organization’s strategic direction, and employment and labour strategies.

 What must you have?

  • Membership, in good standing, with the applicable regulatory body:
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • College of Audiologists and Speech Language Pathologists of Ontario
  • Ontario College of Social Workers and Social Service Workers
  • College of Dietitians of Ontario
  • Minimum 3- 5 years of experience in community health or a related health field.
  • Minimum 3-5 years of experience in management and leadership experience including team building and team management.
  • Knowledge of the health care delivery system and community resources
  • Excellent interpersonal, communication, assessment and decision-making skills
  • Effective time management skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment
  • A valid driver’s license and access to a reliable vehicle
  • Ability to use a computer in a Windows environment
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

  • Certified Health Executive an asset.
  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics.
  • Experience in change management.
  • Experience working in an unionized environment.
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
