The Clinical Lead - Wound Care advocates for the integrated seamless, timely and equitable delivery of high quality, safe, efficient, and cost effective, person-centered, evidence-informed skin and wound care to the people of Ontario Health atHome South West, regardless of the setting.

 

The Clinical Lead-Wound Care will effectively engage and collaborate with hospital, primary and community care partners to provide clinical assessment and direct care intervention to patients with wounds to assess and treat in accordance to best practices in the community.

As a member of the Patient Services team, the Clinical Lead-Wound Care ensures integrated delivery of wound care that spans the continuum of care within Ontario Health atHome. In collaboration with inter-disciplinary team, the Clinical Lead formulates and executes a coordinated strategy for the delivery of wound care based on leading practice.

While some elements of the role are currently being performed remotely, the successful incumbent will be required to regularly report to their assigned office location.

What will you do? 

  • Advise and promotes the ongoing development and maintenance of wound care practices.
  •  Support the development and implementation of wound care best practices including evidence informed guidance and tools to enhance patient experience and outcomes, to support the patient towards increased self-management, treating and/or maintenance at home to avoid ER visits/hospitalizations
  •  Identifies initiative-specific educational needs of partners and staff, and provides learning opportunities to promote knowledge to action.
  • Supports the development and delivery of annual work plans based on priorities set by Ontario HealthatHome South West focused on, but not limited to, effective prevention and management of wound care across
    the system of care.
  • Participates on the multidisciplinary teams, community practice, and operation meetings as it relates wound care to provide clinical assessment in accordance to best practices.
  • Works collaboratively within Ontario Health atHome to address program needs and achieve targets
  • Establishes and maintains effective relationships with patients, service providers and other partners to
    deliver the highest quality patient care.
  • Act as a clinical resource as required, and interpret the role of Wound Care to patients, health care professionals and to the public.
  • Ensure effective co-ordination of services across sectors through ongoing liaison and participation wtih internal and external partners.
  • Engages in creative, collaborative problem solving and planning strategies to provide information on and assist in the resolution of wound healing, including mitigatating delays and service gaps.
  •  Supports the Patient Services Manager in the development of a high performing clinical team for the delivery of Wound Care.
  • Keep abreast with best practices and on the latest tools around skin and wound care education
  • Partners with Patient Service for the development of policies and procedures to ensure quality standards
    are met.

What must you have?

  • Registered Nurse in good standing with their regulatory body
  • Completion of NSWOC certificate OR Master’s level education in the area of wound care
  • 3 to 5 years of relevant experience as a Registered Nurse
  •  Experience working effectively in partnership with broad range of individuals and organizations of diverse backgrounds, preferably within the health care sector.
  • A proven track record of effective working relationships with internal and external customers and clients and the ability to build teams.
  • Strong collaboration skills to participate on inter-disciplinary meetings with colleagues across Ontario Health atHome South West and/or wound care service providers.
  • Ability to deliver information effectively and interpret data clearly in a variety of settings including one-on-one with staff and in team meetings.
  • Ability to deal constructively with a variety of contentious/difficult situations, discuss sensitive information and influence/persuade others to follow a recommended course of action.
  • Exceptional written/oral communication and presentation skills. Must be able to communicate effectively with stakeholders at all levels and to influence change, provide instructions and guidance to staff with respect to activities, challenges and questions. Must be able to explain findings, recommendations and innovations.
  • Excellent interpersonal, diplomacy and presentation skills.
  • Ability to influence decision making through consultation and collaboration when working with internal and external stakeholders.
  • Knowledge of direct care / care coordination models used in community health care organizations
  • Knowledge and application of professional college standards in relation to Case Management/Care Coordination practice
  • Knowledge regarding specialty program(s) assigned
  • Knowledge of tools, systems and databases used in client service delivery and management (e.g., CHRIS, legacy system etc.)
  • Knowledge of South West administrative practices and procedures
  • Ability to use Microsoft Office applications (e.g., Word, Excel, PowerPoint, etc.)
  • Excellent time management and the ability to prioritize multiple conflicting demands.
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.
