What will you do?
- Embody Ontario Health atHome mission, vision and values and apply quadruple aim (enhancing patient experience, enhancing provider/staff experience, improving value and improving populations health) to support continuous quality improvement in daily work
- Utilize quality improvement tools to drive excellence in care and service delivery and to create a culture of continuous quality improvement
- Exemplify, embrace and intentionally promote an inclusive work environment where all are meant to feel they belong
- Continually demonstrate a commitment to create a positive culture of equity, inclusion, diversity and anti-racism
- Maintain with IT the local event reporting system (Tell Us) leading its development, customization and implementation aligned with provincial frameworks
- Provide internal and external (service provider staff) users support including system troubleshooting
- In collaboration with IT, serve as liaison with the software vendor for database issues and development (where applicable)
- Develop and update training material and user resources for the event reporting system
- Conduct orientation for new required internal and external users (e.g. service provider organizations) and lead in the ongoing education of users
- Monitor and audit the reporting of events, identify areas for improvement in reporting, and provide feedback and education to staff as appropriate
- Utilize best practices in analyzing, and trending reported risk events, patient complaints, patient surveys, and other sources of performance data/information, as required
- Provide ad-hoc data, reports, analysis, and interpretation of information, as needed
- Make recommendations about required improvements to event management system or projects
- Other duties as assigned
What must you have?
- Post secondary education greater than 1 year in duration in a related field (e.g. office administration, business, health care, quality assurance, risk management, etc.)
- At least 2 years experience working in a similar role
- Proven ability with statistical and trend analysis and the preparation of formal written reports.
- Strong presentation skills to provide information to managers and staff at all levels of the organization and external community organizations.
- Demonstrated ability to work well with others;
- Demonstrated excellent working knowledge of Microsoft Office (Word, Outlook, Power Point, Excel and Patient Information System programs) with the ability to produce reports with visual additions such as charts, graphs, tables, flow charts and diagrams;
- Demonstrated ability to work in and respond to a dynamic and changing environment;
- Ability to handle sensitive and confidential information in a discreet and professional manner;
- Excellent time management and organizational skills combined with the ability to prioritize and to balance the workload requirements, meeting competing deadlines with minimal supervision;
- Knowledge of home and community care sector through experience working at OHaH or service provider organization;
- Technical knowledge of The Patient Safety Company (Tell Us) event management system
- Fluency in French language preferred.
